Flu Prevention and Vaccination Information
March 25, 2013 - Current information regarding the services being provided by the Yuma County Health District Immunization Department.
Immunizations for Children 18 Years and Younger
$10.00 visit fee, vaccines are included (cash only)
If the child is covered by AHCCCS/KidsCare. you will need to present the ID card or letter
No child will be turned away for inability to pay.
In order for children to receive vaccines you must have and/or meet the following:
• Current Immunization records - Our immunization registry may not contain immunizations given at other clinics, private physicians, or the school district. It DOES NOT contain immunizations given out of state.
• Proof that you reside in Yuma County - The following will be accepted as proof for Yuma County residency: photo ID with current physical address (Drivers License); current utility/cable bill with service address; current rent/mortgage document with physical address; pay stub with physical address; State or local document that can only be obtained with proof of address; Mail with current postmark (cannot be a P.O. Box)
• Guardianship paperwork if you are not the parent - We do not accept notes from parents authorizing a third party, such as a step-parent, aunt, uncle or adult sibling to bring a child in for immunizations. If the child is accompanied by an adult other than the parent/legal guardian they will be given paperwork to take for the parent/legal guardian to read, complete, and sign prior to the child being immunized. The consent must be returned along with a copy of a valid photo I.D. containing a signature for comparison.
Immunizations for Individuals 19 Years and Older
Adult vaccines vary in price; please call the office for the current vaccine price and availability at 928-317-4559.
**We do not offer travel immunization services.**
**Anyone with private insurance is welcome to come to the Health District and the staff will work with them on a case by case basis. (The Health District is working with the Arizona Partnership for Immunizations to procure billing contracts with private insurance companies.) **
Immunization Record Requests
HIPAA regulations prohibit us from faxing records to schools and/or parents or discussing an individual’s immunization records over the phone.
Immunization records can be requested in person Monday through Friday, between 8:00 a.m. and 5:00 p.m. There is a $5.00 charge for copies of a record. There is no charge if you have the record but need it updated.
Immunization records will not be released to a third party such as a step-parent, aunt, uncle, adult sibling or to the parent of a child 18 years and older without proper authorization. Please contact the Immunization Clinic at (928) 317-4559 for additional information on the requirements.
