National Diabetes Prevention Program
The National Diabetes Prevention Program is designed for people that are at risk or have been diagnosed with prediabetes. Having prediabetes does not guarantee a person will get type 2 diabetes. The CDC-recognized lifestyle change program is an evidence-based solution that can help delay/reduce a person’s risk of developing type 2 diabetes through healthier eating and 150 minutes of physical activity per week. In addition, lifestyle change programs can reduce the risk of heart attack and stroke associated with prediabetes and improve participants’ overall health. A CDC-recognized diabetes prevention lifestyle change program can help you build healthy new habits that last a lifetime.
Program is offered in person and virtually. Available in English and Spanish.
Program components:
- CDC-approved curriculum with lessons, handouts and other resources to help you make healthy changes.
- A lifestyle coach, specially trained to lead the program, to help you learn new skills, encourage you to set and meet goals, and keep you motivated.
- A group of people with similar goals and challenges

Diabetes Self-Management Program (DSME)
The Diabetes Empowerment Education Program, also known as DEEP™, is an evidence-based education curriculum designed to help people with diabetes and caregivers gain a better understanding of diabetes self-care. Our team of professionals work with patients on increasing self-management skills by providing practical tools and knowledge to help manage diabetes.
Program is offered in person and virtually. Available in English and Spanish.
Program description:
1. Understanding the human body
2. Understanding risk factors for diabetes
3. Monitoring your body
4. Get up and move! Physical activity and diabetes
5. Controlling diabetes through healthy eating
6. Diabetes complications: identification and prevention
7. Learning about medications
8. Living with chronic disease: mobilizing family and friends
Goals:
Improve and maintain the quality of life of persons with diabetes.
Prevent complications and incapacities
Improve eating habits and maintaining adequate nutrition
Improve the relationship between patients and health care providers.
