You are invited to participate in a survey sponsored by a Center for Disease Control (CDC), Building Resilience Against Climate Effects (BRACE) grant to support the work of the Yuma County Health Department and the Arizona Department of Health Services. The purpose of this project is to study the use of cooling centers and water site in Yuma, Arizona. The survey is being conducted by public health officials with Yuma County and the State of Arizona along with researchers from Arizona State University.
Your response is important. It will help us to understand the opinions of people like you all over Yuma County about their use of cooling centers, and water sites, along with their general experience with heat in Arizona.
The survey on the following pages should take less than 20 minutes to complete. Any resident of Yuma County that is at least 18 years old.
Your answers will be completely confidential and you can skip any questions that you do not want to answer. You are welcome to have help in completing the survey if you have any difficulty reading or writing, but the responses should reflect your experiences and perspectives. Your name and address will not be shared with any marketing firms. You will not be asked to purchase anything. Information from your answers will be grouped together for summary reports. Your name will never be collected or attached to your responses. This study has been reviewed and approved by the Arizona State University Institutional Review Board.
