Community Health Assessment (CHA)
The 2023 Yuma County Community Health Assessment (CHA) was a collaborative effort led by the Yuma County Public Health Services District in partnership with community organizations and stakeholders. Guided by the Mobilizing for Action through Planning and Partnership (MAPP) 2.0 Framework, the CHA identified and prioritized key health issues affecting Yuma County residents. The assessment provided a data-driven understanding of community health needs. Results from the CHA were used to guide the development of the Community Health Improvement Plan (CHIP).
Community Health Improvement Plan (CHIP)
The 2025 Yuma County Community Health Improvement Plan (CHIP) was developed through a collaborative effort led by the Yuma County Public Health Services District in partnership with community organizations, agencies, and stakeholders. This plan reflects the dedication, expertise, and commitment of community partners working together to improve the health and well-being of Yuma County residents. We extend our sincere gratitude to the organizations, agencies, and individuals whose time, knowledge, and experience help set goals, priorities, and strategies that address the community’s most pressing health challenges. The CHIP highlights ongoing progress and goals in three focus areas: chronic disease, mental health, and substance use.
CHIP Monitoring and Evaluation Updates
YCPHSD plans to conduct regular monitoring and evaluation of the YCHIP from January 2026 to December 2028. YCHIP updates will be posted below every six months through December 2028. Subcommittees will continue to engage with each other during the CHIP implementation. YCPHSD staff will contact subcommittees to request updates on action items from the designated leads identified in the action plan. Ongoing updates are shared here every 6 months to highlight progress and ensure accountability.
