Campaign Finance Forms

• Statement of Organization Form       
  • Required if a candidate reaches $1,500 total combined contributions and expenses.
  • Statements should be filed within 10 days of meeting the threshold and can be filed before this deadline, as per A.R.S. §16-906(A).

Fines for Late Filing

• Campaign Finance Reports must be received by our office by the deadline. Any reports received after the due date will incur a $10 per day late fee for the first 15 days. After 15 days, the fee increases to $25 per day. Late fees will continue to accrue until the report is received. Late fees are mandated by A.R.S. §16-937(B).
