Services Included: Birth Certificates: The Office of Vital Records issues birth certificates for those who were born in the State of Arizona from 1950 to present. Birth certificates are issued to those who meet the eligibility requirements, provide a valid government issued Drivers License/ID and pay the $10 fee. We also accept mail-in birth certificate requests at our office.
- Applications are available in our office.
- 1990 to present can obtain a regular birth certificate.
- 1950 - 1990 birth certificates will be issued "as-is".
- For those prior to 1950 you will need to request the birth certificate from the
State Office of Vital Records.
- Birth Certificate Fees: $10.00 each
Death Certificates: We register and issue death certificates that occur in Yuma County. Death certificates are kept in our local office 30 days from the date of registry. Certified copies are issued to those who meet the eligibility requirements, provide a valid Government issued Drivers License/ID and pay the $10.00 fee.

- Death Certificate Fees: $10.00 each
After 30 days all Death Certificates are filed with

Acknowledgment of Paternity: This service is available. We provide the forms in our office. For more information please contact our office at the phone number below.
Fee: $13.00 amending fee plus $10.00 birth certificate fee for a total of $23.00
Amendments/Corrections to Birth Certificates: This service is available for those born 1997 to present. For more information please contact our office.
