Our office is open to the public to order birth certificates or death certificates.
Hours of Operation:
Our customer service lobby is open Monday through Friday from 8 a.m. until 4 p.m. (excluding holidays). However, in order to receive service, you will need to arrive before 4 p.m. All customers who arrive after 4 p.m. will be able to receive information only.
Birth Certificates: Yuma County Office of Vital Records issues certified copies for all Arizona birth events. Birth certificates are issued to those who meet the eligibility requirements, provide a valid signed government issued Drivers License/ID and pay the $20.00 fee. Birth certificates requested in person are processed 3 business days after they are requested, and 10 business days if requested by mail. Applications are available in our office or can be downloaded from the State Office of Vital Records website at www.azdhs.gov
Death Certificates: Yuma County Office of Vital Records issues certified copies for all Arizona death events. Certified copies are issued to those who meet the eligibility requirements, provide a valid signed government issued Drivers License/ID and pay the $20.00 fee. Death Certificates requested in person are processed 7 business days after they are requested, and 10-15 business days if requested by mail. Applications are available in our office or can be downloaded from the State Office of Vital Records website at www.azdhs.gov
Acknowledgment of Paternity (AOP): This service is available through our office. The form may be used to establish paternity for a child under the age of 18, if both parents are in agreement to the identity of the biological father. Parents may choose to complete the Acknowledgement of Paternity at the hospital when the child is born or may do it at a later time. In either case, the father’s name will not be added to the birth certificate until the form is filed with the County or State Office of Vital Records, or the Hospital Paternity Program. These form(s) are available in our office or may be downloaded from www.azdhs.gov A $30.00 processing fee is required to amend the birth record. Amendments/Corrections to Birth and Death Certificates: This office makes eligible corrections or amendments to Arizona birth certificates from 1997 births to the present. Eligible corrections or amendments to Arizona death certificates are made to death events from 2008 to the present. The process to make changes to a birth or death certificate varies depending on the type of legal change needed and how recently the record was filed. Please call before coming to our office and find out what documentation will be required. A $30.00 processing fee is required to correct or amend a birth or death record.

Yuma County Office of Vital Records does not accept online orders or credit/debit cards as payments; however, for your convenience, an optional expedited service fee is available to those who wish to obtain same day service for birth certificate requests ONLY.
