The Yuma County Office of Emergency Management (OEM) has developed this pre-registration process to establish an assistance registration database. This database will contain information about those individuals with acute or functional needs such as mobility issues, vision or hearing issues or perhaps those individuals who do not have the ability to drive – all issues that could hinder their ability to self-evacuate during an emergency situation.
The Office of Emergency Management will reference the pre-registration information to assist emergency personnel in knowing that an individual at a specific location may need some special attention during an emergency.
We also recommend printing and forwarding this form to individuals who may require assistance and may not have Internet access. Please mail the completed form to the following address:

The Office of Emergency Management and departments under the County Administrator take personal privacy and confidentiality very seriously.
